Lutheran Home at Kane Receives Age-Friendly Designation

Kane, Pennsylvania - The Lutheran Home at Kane has achieved special recognition as an Age-Friendly Health System — Committed to Care Excellence from the Institute for Healthcare Improvement (IHI).  The Lutheran Home at Kane has focused its efforts to ensure age-friendly care for all the residents they serve. Over the past five months, detailed information has been gathered on all residents under The Lutheran Home’s care.  The care being provided is set on evidence-based elements of high-quality care, known as the 4Ms.   

The essential 4M elements are: 

  • What Matters: We identify and align care with each resident's specific health outcome goals and care preferences including, but not limited to, end-of-life care.

  • Medication: If medication is necessary, we use Age-Friendly medications that do not interfere with What Matters to the individual.   

  • Mind: We prevent, identify, treat, and manage dementia, depression, and delirium.

  • Mobility:  We ensure residents move safely every day to maintain function and do What Matters.

The Lutheran Home at Kane is proud to join the ranks of other great organizations who are putting the 4Ms into practice. “Our commitment to quality care and learning only grows stronger as we further enhance our efforts to improve the delivery of care we provide to our most vulnerable population.  We have always been committed to care excellence and we are proud and humbled to have achieved this external recognition” said Charlotte Floravit, Chief Executive Officer of The Lutheran Home at Kane.  “The Age-Friendly Health System’s initiative continues to be an important part of our overarching vision to provide every resident with the best care possible. Above all, we aim every day to earn recognition of excellence from the population we serve.

This designation would never have been possible without the involvement of the entire organization. A special thank you to Marie Goode, Director of Nursing, whose drive and determination made this initiative a success.  We are excited to continue our ongoing commitment towards this reliable practice of the 4Ms.” stated Floravit.
